What is HACC?
HACC stands for Home and Community Care. Home and Community Care aims to help frail aged people and people with a disability to be independent and live at home for as long as possible, rather than being admitted to a nursing home or hostel.
A range of community and government organizations in the Central West offer HACC services.
Who may use HACC Services?
Anyone of any age, who has a disability that makes independent living difficult, can apply for HACC services. That is, people, including those with dementia or from different cultural backgrounds, who have difficulty carrying out any of the task of daily living (eg. dressing, cooking, travelling, showering etc). People using HACC services are often called clients or consumers.
HACC services are also available to carers. Carers are the family members, friends or neighbours who assist the frail older person or person with a disability to live independently.
However, people who are receiving home support services that are funded by another government program are generally not eligible eg. Attendant Care.
What do HACC services Provide?
• Meals
A range of food services can be provided both in your home and in a community centre.
• Home Care
A range of services including personal care, showering, dressing, household chores and shopping can be provided.
• Home Maintenance and Modification
The Home Maintenance and Modification Service can do small repairs, maintenance and larger modification jobs to make your home safer and more accessible.
• Transport
Transport can be arranged to take you shopping, get you to appointments, visit day centres and go on group social activities.
• Respite Care
A number of services can provide a break for Family and Carers e.g. social activities in the home or at a centre away from home.
• Health Care
In the Central West health care services including community nursing and some occupational therapy may be available.
• Support Services & Neighbour Aid
A range of support services including carer support groups, Neighbour Aid services, home visiting and companionship can be provided.
• Community Options
These are case management services for clients with complex needs and may include respite care, personal care, home help and transport.
